What's Involved?
Simply call us and talk with a New Guest Consultant about your medical concerns. Our New Guest Consultant will then discuss with you the various treatment options available to you through Hotze Health & Wellness Center using natural, bioidentical hormone therapy and our Treatment Program. It is the first step in helping you decide if the program is right for you.

On average, these telephone sessions take between 30 minutes to 1 hour.

STEP 2: Schedule an Appointment
What's Involved?
After you have decided that Hotze Health & Wellness Center is right for you, the next step is scheduling your appointments. There are two appointments that can be scheduled at the time of your call. They are as follows.
1. MEDICAL HISTORY QUESTIONNAIRE – Can be done online or over the phone.
2. EXAMINATION AND TESTS (OFFICE)
How Long Does Each Appointment Take?
The online medical history questionnaire can take from 10-30 minutes. Your initial examination can take from 3-5 hours.

STEP 3: Medical Questionnaire
What's Involved?
You will be asked to enter your medical history into an online questionnaire. If you do not have access to a computer, then a detailed questionnaire can be emailed, faxed or mailed to you prior to your discussion with the Questionnaire Consultant to give you the appropriate amount of time to review the questions and fill in the answers.
It is important that you are open with your Questionnaire Consultant so that your physician will know exactly what symptoms you are experiencing and understand your complete medical history.

30 – 45 minutes.
How Much Does It Cost?
There is not an itemized charge for this service as it is included in the cost of your examination.
STEP 4: Wellness Evaluation
What's Involved?
Upon your arrival at Hotze Health & Wellness Center, you will be greeted at the door by a Hotze Health & Wellness Center representative and escorted to guest check-in. A nurse will then take you to a private room where your vital signs and blood tests will be performed.
For your comfort and convenience, a snack plate and a choice of beverage will be served immediately after, as the blood test requires a 6-hour fasting period.
A series of tests will then be performed based upon your health history and concerns. These tests may include allergy testing, a bone density scan, and an EKG.
Upon completion of your tests, you will receive a thorough physical examination by one of our physicians and a one-on-one consultation to review your medical history and exam.
Based upon this examination, the doctor will discuss and explain the all-natural medical options available to you and the recommendations he would prescribe tailored to your needs.
How Long Does It Take?
Please allow 3 to 5 hours. Actual time will depend on the amount of tests needed and consultation time (physician, pharmacist and vitamin consultant) to explain your treatment program.

Because the program is tailored to your specific needs, the cost can vary depending upon which parts of our regimen will best benefit you. Our Wellness Programs can be discussed with your New Guest Consultant for a more detailed estimate based upon your medical needs.

STEP 5: Care Plan Review
What's Involved?
You will have your Care Plan Review with a nurse three weeks after your medical examination and tests to review your lab results and to further explain the treatment program your doctor has prescribed for you. If you have any additional questions about your report, you may contact our office to speak with a nurse who will be able to answer any questions that you may have and help guide you on your new healthy lifestyle.
