STEP 1: Consultation
What's Involved?
Simply call the Hotze Health & Wellness Center and talk with a new guest consultant about your medical concerns. Our consultant will then discuss with you the various treatment options available to you through the Hotze Center using natural, bio-identical hormones and our Eight-Point Treatment Program. It is the first step in helping you decide if the program is right for you.
How Long Does It Take?
The average counseling session takes between 30 minutes to 1 hour.
How Much Does It Cost?
There is no cost and no obligation to visit with a new guest consultant.
STEP 2: Schedule an Appointment
What's Involved?
After you have decided that the Hotze Health & Wellness Center is right for you, the next step is scheduling your appointments. There are three appointments that can be scheduled at the time of your call. They are as follows.
1. MEDICAL HISTORY QUESTIONNAIRE – Done over the Phone
2. EXAMINATION AND TESTS (OFFICE)
3. PHONE FOLLOW-UP

10-15 minutes.
How Much Does It Cost?
There is no cost to make an appointment. It is a necessary step, however, since the initial evaluation and office visit at the Hotze Health & Wellness Center must be scheduled in advance.
STEP 3: Medical Questionnaire - Done over the phone
What's Involved?
A Questionnaire Consultant will call you to review your medical history.
A detailed questionnaire will be emailed, faxed or mailed to you prior to this discussion to give you the appropriate amount of time to review the questions and fill-in the answers.
It is important that you are open with your Questionnaire Consultant so that your physician will know exactly what is going on inside your body.

30 – 45 minutes
How Much Does It Cost?
There is not an itemized charge for this service as it is included in the cost of your examination.
STEP 4: Examination
What's Involved?
Upon your arrival at the Hotze Health & Wellness Center, you will be greeted at the door by one of our representatives and escorted to guest check-in where your vital signs and blood tests will be performed.
For your comfort and convenience, a snack plate and a choice of beverages will be served immediately after as the blood test requires a 12-hour fasting period.
A series of tests will then be performed based upon your health history and concerns. These tests may include allergy testing, a bone density scan, an EKG, and a heart scan.
Upon completion of your tests, you will receive a thorough physical examination by a one of our physicians and a one-on-one consultation to review your medical history and exam.
Based upon this examination, the doctor will discuss and explain the all-natural medical options available to you and the recommendations he would prescribe tailored to your needs.
How Long Does It Take?
Please allow 3 to 5 hours. Actual time will depend on the amount of tests needed and consultation time (physician, pharmacist and dietician) to explain your treatment program.
How Much Does It Cost?
Cost varies from $780 to $3,900, according to the number of tests performed and treatment plan. Your New Guest Consultant will be able to provide you with a more detailed estimate based on your medical needs.
STEP 5: Phone Follow-Up
What's Involved?
This consultation is generally scheduled three weeks after your medical examination and tests to review your lab results and to further explain the treatment program your doctor has prescribed for you. This consultation is conducted by a registered nurse who will be able to answer any questions that you may have and help guide you on your new healthy lifestyle.
