Your care plan and associated costs depend on the services recommended for your individual needs.
Each program has an initial visit fee based on the care determined to be appropriate for you.
In addition to your initial visit fee, new guests join our membership for $100 per month, billed to your credit card. Membership is required because consistent follow-up is how our guests get results, and it makes that follow-up simple and affordable.
Your membership includes:
● Complimentary office visits, including your required rechecks (typically 3–4 in year one), sick visits, and any additional visit your provider recommends
● 15%* off, every day, on bloodwork, compounded prescriptions, and vitamins and supplements
● Savings at all three locations*: Hotze Health & Wellness Center, Physicians Preference Pharmacy, and Physicians Preference Vitamins
*Exclusions apply, including devices and select services, products and compounded prescriptions.
As is common in functional medicine, we are a cash-based practice and do not contract with insurance companies. We believe your provider, not an insurance company, should decide your treatment plan. You will receive an itemized bill at the end of each visit to submit to your insurance company if you choose. Coverage varies, so we recommend checking with your carrier.
