8 Modifiable Risk Factors for Cognitive Decline

A modifiable risk factor is a condition or behavior you can change, unlike your age, sex, or genetic makeup. Researchers distinguish these factors because they represent real opportunities for prevention. Instead of viewing cognitive decline as inevitable, current science treats it as the cumulative result of choices, exposures, and untreated conditions that build over decades. 

The nine modifiable risk factors for cognitive decline we cover in this article include:

The landmark SPRINT MIND trial followed more than 9,300 adults aged 50 and older with hypertension and found that intensive blood pressure control significantly reduced the combined rate of mild cognitive impairment and probable dementia.² Notably, researchers observed that this protective effect persisted for years after the intensive treatment period ended. 

Because hypertension often develops silently, regular monitoring matters well before symptoms appear. A meta-analysis of prospective cohort studies involving more than 726,000 participants found that hearing loss was independently associated with a substantially elevated risk of dementia, even after researchers adjusted for other health factors.³ Untreated hearing loss may accelerate cognitive decline through several pathways, including reduced auditory stimulation to the brain, increased cognitive load during listening, and greater social withdrawal.

Type 2 Diabetes and Brain Health 

Diabetes affects far more than blood sugar. Chronic high glucose levels damage blood vessels throughout the body, including those in the brain, and contribute to inflammation and insulin resistance in brain tissue. A large cohort study found that individuals diagnosed with type 2 diabetes before age 50 faced more than three times the risk of all-cause dementia compared to those without diabetes, with elevated risk persisting even when diagnosis occurred later in life.⁴

The encouraging news is that physical activity remains one of the most accessible tools for lowering cognitive decline risk factors at any age. Even moderate, consistent activity, such as brisk walking, offers measurable benefit. 

Obesity, Depression, Isolation, and Vision Loss 

The remaining risk factors on the Lancet Commission’s list each carry independent weight, and together they round out the full picture of modifiable risk factors for cognitive decline. The table below summarizes how each one contributes to risk.
